Tani starred in director Kōyū Ohara's Nikkatsu Roman pornos, Fascination: Portrait of a Lady (1977), Rope Hell (1978) and Fairy in a Cage (1977) which had Tani playing the role of a wealthy business woman who is tortured by the sadistic head of the inquisition branch of the Japanese military during World War II. Director Shōgorō Nishimura's 1978 film, Lady Black Rose, contained two notorious scenes which are often selected as examples of the Roman porno S&M genre. In one, gallons of water are forced into Tani's mouth through a funnel while her stomach can be seen enlarging. Tani points out that scenes like these were often tricks, though no special effects were involved.

Tani would work with director Ohara again in Fascination: Portrait of a Lady (1977), and Rope Hell (1978), which also reunited her with Hirokazu Inoue, one of her torturers from Fairy in a Cage. In contrast to these torture-themed films, Ohara had been successful with light, youth-centered comedies, such as his later Pink Tush Girl series. In a review of the films of Kōyū Ohara for the journal Asian Cult Cinema, Graham R. Lewis notes that Ohara uses conflict between social classes as a theme in Fairy in a Cage. Both Lady Kikushima and Judge Murayama are members of the upper class, while Taoka, the new recruit ordered to torture Kikushima, is from the lower class.

Ohara had previously worked with Naomi Tani in Fascination: Portrait of a Lady (1977), and had teamed her with her on-screen tormentor in Rope Hell, Hirokazu Inoue in Fairy in a Cage (also 1977). Both of these films had also based on Dan's writings. Allmovie judges Rope Hell to be an inferior film compared to Fairy in a Cage. In their Japanese Cinema Encyclopedia: The Sex Films, the Weissers also write that Rope Hell is not up to the quality of Ohara and Tani's previous work together, but comment positively on Ohara's visuals.
