Convinced that the PAVN had returned in strength across the DMZ, on 14August the 4th Marines commander Colonel Cereghino moved the 1st Battalion, 4th Marines up from Phu Bai Combat Base to relieve the 2/4 Marines from their defensive duties to conduct a reconnaissance in force along Route 9 from Cam Lo to the Rockpile and the area north.

In May 2004, the battalion with the 11th Marine Expeditionary Unit, the battalion again returned to combat in Iraq for a 9-month deployment, which was highlighted by a major victory against insurgent forces in Najaf, after the fighting at Najaf cemetery, Bravo Company guarded Abu Ghanrib prison and supported Operation Phantom Fury.
A patrol by Combined Anti-Armor Team (CAAT) Alpha, Weapons Company, 1st Battalion, 4th Marines (1/4) approached a maternity clinic located directly across the street from the home of Muqtada al-Sadr on the outskirts of the city.
