She leads Spider-Man across several crime scenes in New York that were the work of a Maggia Enforcer and included audio recordings of her and one of her officer's attempts to take the criminal down. In the game's main storyline, the super-human crime lord Mr. Negative orchestrates a plot to seize control of New York City's criminal underworld. In his civilian life, Parker is supported by his Aunt May (Nancy Linari) who volunteers at the F.E.A.S.T. homeless shelter run by philanthropist Martin Li (Stephen Oyoung).
Mister Negative appears in Spider-Man, voiced by Stephen Oyoung. While operating as the benevolent head of F.E.A.S.T., he and his Inner Demons seek to take control of Kingpin's territory after the latter was defeated by Spider-Man and imprisoned. It is also revealed that Mister Negative got his powers from the Devil's Breath virus that killed his parents and led to Li's vendetta against Mayor Norman Osborn, who was responsible for the accident. During an attack on Mayor Osborn's rally, one of his suicide bombers killed Jefferson Davis as Mister Negative blasts away Spider-Man. He makes Aunt May the new head of F.E.A.S.T prior to fully committing to destroy Osborn. Even though Mister Negative is defeated by Spider-Man and arrested by the police, he is freed from prison by Otto Octavius who has since become Doctor Octopus, and who also seeks revenge against Mayor Osborn, and is recruited into the Sinister Six. At the location where the cure for the Devil's Breath virus is, Spider-Man defeats Mister Negative after a failed attempt to talk him down as Li's hatred for Osborn was too strong. Doctor Octopus then arrives and knocks out Mister Negative, deeming him useless, before stealing the cure and escaping with Mayor Osborn. It is unknown what happens to him following Octavius' imprisonment, though it is likely he may also have been put in custody.

In the course of the storyline, she is one of several victims of a lethal Devil's Breath virus unleashed by Doctor Octopus as part of his plans to ruin Mayor Norman Osborn. Peter is forced to let her die even after finding a cure because there wasn't enough time to mass-produce the cure to use it on her as the scientists needed the entirety of the sample to perfect it for large-scale use. May dies after admitting that she has known for a while that Peter is Spider-Man and telling her nephew she's proud of him. At the cemetery, Aunt May is buried next to Uncle Ben's grave as Peter and Mary Jane attend her funeral; her tombstone stating too that she was born in 1953 and died in 2018. At F.E.A.S.T., there is a standing commemorative plaque in her honor.
